Sick Notes and Missed Appointments
Sick Notes Policy
If you need a sick note for a minor illness that has already resolved, please do not schedule a provider appointment — this allows us to reserve appointments for students with active health concerns.
Instead, download the “Class Excuse” form available on our Forms page.
Missed Appointment Policy
If you can’t make it to your appointment, please call at least 30 minutes in advance to cancel or reschedule. A $20 no-show fee will apply if you don’t notify us.
