We're proud of our rural heritage in Montana. As part of our mission to recruit physicians to practice in underserved parts of the state, we offer robust rural training experiences focused on full-spectrum medicine.
Our program recently ranked second in the nation among the residency programs surveyed by the Rural Training Track Collaborative for graduating the most family physicians that go into rural practice.

Anaconda is nestled in the heart of Southwest Montana, offering easy access to a variety of outdoor activities, including hiking, fishing, skiing, and biking. While the town provides a scenic and adventurous lifestyle, it is also conveniently located just 30 minutes from Butte and about two hours from Missoula and Bozeman, where residents can find urban amenities. With a population of 9,673, Anaconda prides itself on its strong sense of community. Originally established as a copper-smelting boomtown in the late 1800s, it played a crucial role in processing copper mined in Butte. Over time, Anaconda developed a rich industrial heritage, shaped by its blue-collar roots and diverse cultural influences. One of the town’s most recognizable landmarks is its towering smokestack, which rises against the skyline as a cherished symbol of its historic past. However, Anaconda’s mining legacy also brings environmental considerations, particularly due to its proximity to a Superfund site.

Community Hospital of Anaconda (CHA) has been serving the healthcare needs of Anaconda-Deer Lodge County (ADLC) since 1889. The hospital is a privately owned, non-profit, twenty-five bed critical access facility built in 1981. CHA offers a full continuum of care including inpatient/outpatient hospital services, physical, occupational and speech therapy, advanced imaging, oncology/hematology, urology, neurosurgery, general surgery, orthopedics, rheumatology, endocrinology, obstetrics, family medicine, long-term care, and Hospice/Home Health services.
In addition, CHA offers a full range of visiting specialists, alleviating the need for members of our community to travel outside Anaconda for specialized services. Located in Southwest Montana, CHA has a medical staff of over 60 physicians and 400 employees that support the healthcare needs of our region

Located in the western portion of the 1.5 million acre Blackfeet Indian Reservation where Browning is home to over 7,000 descendants of the Ampska Pikuni Nation and bordering the crown jewel of our nations National Park System, Glacier National Park, the hospital picturesquely sits in the shadows of the "Backbone of Mother Earth" or what is more commonly known as the "Shining" Mountains of the rugged Northern Rockies. Several outdoor activities abound within a short driving distance including world class fishing, hiking, camping, boating and skiing.
First opened in Browning, Montana in1937, the Blackfeet Community Hospital has since been transformed into an expansive modern day 110,000 square foot 28 bed comprehensive health care facility complete with a 64-slice state of art computer tomography (CT) unit within a fully digitized radiology and lab service department. The hospital offers a full-time diabetic clinic with daily Podiatry consultations and boasts of a sophisticated 8 bay emergency room equipped with a rooftop helipad transfer complex along with an urgent care/ same day appointment clinic delivering emergent and urgent care for slightly more than 131,000 patient encounters per annum. The facility contracts with other larger medical centers within the northwest to meet the needs of those patients experiencing severe trauma and or requiring critical care.
Outpatient dental and optometry services are available five days per week. The facility also has in-patient pharmacy capabilities including an automated outpatient pharmacy department filling up to 1200 prescriptions per day.
A daily Women's Health Center providing pre / post-natal care five days per week and a 24/7 OB/GYN department averages 200 to 210 newborns per year. An OR department provides surgical services for both outpatient and inpatient procedures with a full-time general surgeon and contracts with other sub-specialties.

The Southwest Montana Community Health Center has been serving southwest Montana by providing medical, dental, behavioral health and pharmacy services to people of all ages since 1986. Their mission is to inspire hope and empower wellness by providing access to comprehensive healthcare. Confederated Salish Kootenai Tribal Health serves the Flathead Indian reservation which is located directly north of Missoula and encompassing the southern half of Flathead Lake. With 1.3 million acres, the reservation is made up of diverse landscapes, including the Jocko Valley and the Bison Range. There are also numerous communities – like Polson, Ronan, Charlo and St. Ignatius, to name a few – on the reservation.

Deer Lodge Medical Center is a non-profit 501c3 community hospital located in Deer Lodge, Montana. Managed by Cypress Healthcare, Deer Lodge Medical Center is a certified critical access hospital serving Powell County and the outlying communities.They offer inpatient and outpatient care, 24-hour emergency care, physical therapy, general surgery, chiropractic care and a broad array of specialty services supported both through the hospital and clinic.

Dillon, the county seat of Beaverhead County, is a peaceful and affordable town in a vast yet sparsely populated region. Surrounded by the Ruby Valley and rivers like the Big Hole and Beaverhead, it offers numerous recreational activities that attract visitors year-round, including fishing on the Beaverhead River, snowmobiling, hiking, four-wheeling, skiing at Maverick Mountain Ski Area, and attending the annual Demolition Derby. They offer preventative services, acute visits, prenatal care, pediatrics, minor-surgical procedures, disease management services, radiology, recovery and rehabilitation servies, outpatient IV therapy, and 24-hr emergency and trauma care.

Eureka is in the Tobacco Valley in North Lincoln County. Located approximately 50 miles from Whitefish, MT. Population of Eureka is over 1,500, however our service areas consist of over 5,000.

We started as a one provider clinic in the mid-1990s and have since grown to seven providers (2 physicians, 3 nurse practitioners and 2 physician assistants).
Eureka is 45 minutes away from the nearest hospital, Logan Health Whitefish, and 75 minutes from Logan Health in Kalispell, MT. We have no local ER so serve not only as a traditional Family Practice Clinic but also accept walk in patients to address their urgent needs, same day appointments and stabilize emergent patients for ALS transport, as necessary. All our providers are ATLS and ACLS trained, and all nurses are ACLS trained. We also provide care for our local 45-bed nursing home and two assisted living facilities and hold a clinic once a week at the local public school.
Our providers practice in Eureka because they like the work-life balance living in a rural mountain town affords them. Eureka is a class B school system usually graduating 70-100 students per year and offers small class size yet big enough to have advanced curriculum for our students to be competitive as they graduate. For skiing we are 1.25 hours from Whitefish Mountain, 1.5 hour from Turner, 1 hour from Fernie and 1.5 hour from Kimberly. We cross country ski and snowshoe out the doors of our homes. Glacier Park is 1.5 hour away, but we hardly go there as we have premier hiking out our back door with no crowds in the Ten Lakes Wilderness area. “Stone Hill” is a destination rock climbing site on Lake Koocanusa in our back yard. We have numerous lakes for water activities and premium fishing along with the Elk River and Kootenai River for fishing, kayaking, and rafting. Eureka is also on the Continental Divide Mountain Bike Trail that runs from Fernie to Mexico and just one of the areas to enjoy cycling on or off pavement.
Our providers enjoy full spectrum clinical medicine without the obligations of a hospital. We routinely work-up and stabilize acute coronary syndrome for air transfer to Kalispell and regularly utilize our full ACLS skills. Thankfully, we do not have to use our ATLS skills much but do get gunshot wounds that walk through our doors along with a fair share of chain saw injuries and ATV/MVA accidents. We otherwise offer in-office circumcisions, advanced contraception options, minor surgical procedures along with vasectomy.
Logan Health Rehab-Eureka offers full spectrum physical and occupational therapy.
We have a variety of specialty providers seeing patients in Eureka regularly, both in person and via telehealth. We have a mobile mammography coach on-site two days per months offering 3D mammography screening and DEXA.
We are excited to host residents. We have been hosting medical students since 2000 and are looking for adventurous personalities to help us “break the trail” in getting a residency rotation site up and running.

Located in the Bitterroot Valley, Hamilton is the county seat of Ravalli County and sits approximately 45 miles south of Missoula Montana. With a population of around 5,000, the City of Hamilton has a small‐town feel with amenities to serve the greater area of Ravalli County with a population of 44,876 as of the 2020 Census. The town of Hamilton is the central trade area for hundreds of small farms, ranches and orchards that fill the valley. Hamilton if home to the Daley Mansion built in the late 1800s for Marcus Daly, one of Montana’s colorful “Copper Kings”.

Daly Hospital is a “non-profit” rural critical access hospital with 24 acute care beds and over 90 medical providers. Throughout the Bitterroot Valley, there are an array of outpatient medical services, surgical specialties, medical clinics, and primary wellness and preventative care available.
As a standard, residents are scheduled in Emergency, Internal Medicine (Hospitalists) and Primary Care (our primary care clinics are in Hamilton, Corvallis, Stevensville). We have Urgent Care within our primary care clinic as well. Specialties: OB/GYN, Orthopedics, Neurology, Neurosurgery, Pain Clinic, Dermatology, Nephrology, Cardiopulmonary, General Surgery, Urology, Ophthalmology, GI, ENT, Oncology/Infusion, Sleep Center, and a Specialty Clinic with providers that rotate through.

Lewistown is a family-friendly community that combines the ambiance of Montana’s natural beauty with the authenticity of our western lifestyles. The area was the territory of the Blackfoot Native Americans. In 1874, Fort Lewis was established there by Company "F" of the 7th U.S. Infantry to provide military protection for people traveling on the Carroll Trail, then the shortest route between Carroll, Montana and Helena. Lewistown is named after this fort. Lewistown is a city in and the county seat of Fergus County, Montana, United States. The population was 5,952 at the 2020 census. Lewistown is in the geographic center of the state, southeast of Great Falls (95 miles), northwest of Billings (97 miles) and 122 miles from Bozeman.

Central Montana Medical Center has been recognized as a 2025 Top 100 Critical Access Hospital by The Chartis Center for Rural Health. Chartis’ annual Top 100 award program recognizes outstanding performance among the nation’s rural hospitals based on the results of the Chartis Rural Hospital Performance INDEX®. CMMC has won this honor three years in a row. The hospital is currently under construction with a new Lobby and cancer center set to be completed in early summer 2025. -

Plains is a town in Northwest Montana with the population of 1106 in the 2020 census. This is a rural community that straddles the Clark Fork River. Plains is approximately 80 miles NW of Missoula, MT. This town presents quiet Western Montana living with great outdoor recreation opportunities.

Welcome to Clark Fork Valley Hospital and Family Medicine Network, a not-for-profit community healthcare organization providing services to patients since 1971 in Plains, Montana. Clark Fork Valley Hospital is a 16-bed critical access facility serving the residents of Sanders County and the surrounding area. We additionally manage a long-term care facility with 28 resident beds.

Plains Family Medicine, located at the main hospital campus in Plains, MT provides primary and specialty care services to the residents of Plains and the surrounding communities. Clark Fork Valley Hospital and Family Medicine Network offers a very sophisticated medical environment while maintaining its small-town feel. With a team of hospitalists and several specialty outreach providers, including an orthopedist and a general surgeon, three rural health clinics countywide, a home oxygen service, visiting nurse program and hospice service, Clark Fork Valley Hospital projects a larger footprint than one would expect of a rural Critical Access Hospital.
Our facility is a 16-bed Critical Access Hospital. We offer learning opportunities in inpatient medicine, rural emergency medicine, general surgery & endoscopy, outpatient family medicine, and some obstetrics.

Polson, a picturesque city on the southern shore of Flathead Lake, sits at the foot of the Mission Mountain Range within the Flathead Indian Reservation. As the largest freshwater lake in the West, Flathead Lake attracts thousands of visitors each summer. However, economic disparities exist among year-round residents, with household incomes averaging about $20,000 below the national median. Polson serves as the county seat of Lake County, with a population of 5,357—a number that rises significantly during Montana’s peak tourist season. The city is located approximately 70 miles north of Missoula, 50 miles south of Kalispell, and 70 miles south of Glacier National Park.

St. Joseph Medical Center is a “non-profit” rural critical access hospital with 22 acute care beds. There are an assortment of outpatient medical services, surgical specialties, medical clinics, and primary wellness and preventative care available. EPIC is used for the EMR and the hospital shares leadership and policy with St. Patrick Hospital in Missoula.

Nestled amid the stunning Mission Mountains and abundant fishing and wildlife habitats, this vibrant community of over 3,000 residents offers a rich array of resources and recreational opportunities. First settled by Salish residents in 1883, the town was originally named Spring Creek after the local warm springs that feed into the nearby Flathead River. In 1893, residents renamed it Ronan Springs. The town saw rapid growth in 1910 when the federal government opened the Flathead Indian Reservation to non-Indigenous homesteaders.
